SEVP Portal Reset Request Form

This form is for F-1 students on approved Optional Practical Training or the STEM extension who need to have their SEVP Portal account reset. You may need your portal to be reset if you have locked yourself out of your SEVP account or your log-in credentials have expired.

Upon completion of this form, the HIO will process your request. You will then receive an email from SEVP, not the HIO, to the email address indicated in this form. Please be mindful of spam/junk folders as well.

Please note that Designated School Officials (DSOs) have limited functionality in the Student and Exchange Visitor Information System (SEVIS) regarding the SEVP portal. Additional SEVP portal assistance is also available on Study in the States.
